Agreement to Terms
By accessing and using the services provided by Craig Wellness LLC and Lauren Craig, LCSW, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.
Services Provided
Craig Wellness LLC provides the following services:
- Individual counseling and therapy sessions
- Virtual and in-person therapy services
- Specialized support for moms and women
- Treatment for grief, anxiety, and life transitions
- Mind-body integration therapy
Professional Qualifications
Lauren Craig is a Licensed Clinical Social Worker (LCSW) in the state of Oklahoma (License #20397) and a Registered Yoga Therapist (RYT-200). All services are provided in accordance with professional standards and ethical guidelines.
Appointment and Payment Terms
Scheduling
- Appointments must be scheduled in advance
- 24-hour notice is required for cancellations
- Late cancellations may result in a fee
- No-show appointments will be charged the full session fee
Payment
- Session fee: $150 per session (cash pay)
- Payment is due at the time of service
- We accept Blue Cross Blue Shield and Health Choice insurance
- Sliding scale fees may be available based on need
Confidentiality and Privacy
All therapy sessions are confidential. Information shared during sessions will not be disclosed without your written consent, except in the following circumstances:
- Risk of harm to yourself or others
- Suspected child or elder abuse
- Court order or legal requirement
- Insurance billing and authorization
Limitations of Service
Please understand that:
- Therapy is not a substitute for medical care
- Results may vary and are not guaranteed
- Emergency situations require immediate medical attention
- Virtual sessions require a stable internet connection
Emergency Situations
If you are experiencing a mental health emergency, please:

Website Use
By using our website, you agree to:

Craig Wellness LLC and Lauren Craig, LCSW shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services or website.

Either party may terminate therapy services at any time. We will provide appropriate referrals and ensure continuity of care when possible.
Governing Law
These terms are governed by the laws of the state of Oklahoma. Any disputes will be resolved in Oklahoma courts.
